Dean's words might as well be a knife piercing Mary's already fragile heart. She knows that it's different for him without Sam - it's different for her, too - but they haven't talked about that. She was expecting him to be a little less blunt if it ever did come up, so she winces instinctively, immediately trying to recover and act like that didn't bother her at all.
